The Community Health Partnership (http://bit.ly/NTeC0W) will serve frequent users of health care services in the East Baltimore community and some patients admitted to Johns Hopkins Hospital and Johns Hopkins Bayview Medical Center. By July 2015, nearly all adults admitted to Hopkins Hospital and Hopkins Bayview will experience improved care and smoother transitions when returning home or to another care facility, such as a skilled nursing facility.
